Because the goal of the STOP program is to transform the way that colorectal cancer screening is delivered, the program was designed to be sustained. Rather than building a stand-alone tracking system, we built a population management system directly into the electronic health record. The care that patients receive at the clinic will therefore complement and reinforce this automated program.

Health Plan Models
There are two different models used the health plans that participated in the BeneFIT study. One chose a centralized model due to their organizational structure, the other health plan chose a collaborative model due to their organizational structure and relationships with clinics/health systems. These documents help explain the two different models, their advantages and challenges, and the resources needed to implement a mailed FIT program.

- Introductory Letter – Copy of the introductory letter sent to each enrollee eligible for screening
- Kit Letter – Copy of the letter sent with the FIT kit to enrollees
- Incentive Letter – Copy of the letter sent with the FIT kit to enrollees that includes information about an incentive
- Reminder Card – Copy of the reminder card sent to enrollees to remind them to return the FIT kit
